First Visit & New Patients
Please bring the following to your first appointment:
- Drivers license or government-issued identification
- Referral form from your dentist
- Insurance card
- Any necessary Insurance Forms, completed, if applicable
- List of your Current Medications
- X-Rays or scans - Forwarded to us by your referring dentist if applicable
- New Patient forms need to be completed before your visit via the link that our staff will send you by text or email
For your convenience, after you schedule your first appointment with our office, you will receive a text to complete all of your new patient forms easily from any device. No need to print the forms, they will be automatically submitted to us.
